Registration fee covers:

- Delegates are encouraged to pre-register by mail or fax as space is limited. All participants making scientific presentations MUST be pre-registered. Instructions are provided on the enclosed form (copies permissible). Early registrants will benefit from a reduced fee until May 1, '97. Persons wishing to register after August 28, '97 may do so on-site (space permitting). Applications should be forwarded to the following administrative secretariat (Japan Travel Bureau, Inc.):

Payment in Japanese Yen is due at the time of registration. Payments can be made by a) Bank check, b) Bank transfer or c) Credit cards (Visa, American Express, Diners or Mastercard).
- a) Bank checks must be drawn on a bank with a Japanese branch and made payable to "Japan Travel Bureau." (Personal Checks will NOT be accepted)

- Delegates paying by bank transfer MUST send a duplicate or photocopy of the transfer order with their registration form. The name and address of the delegate should appear clearly on the bank transfer. Please note that all charges due on international bank transfers must be paid by the registrant.
- c) Delegates paying by credit cards (VISA, American Express, Diners or Mastercard) must add 3.5% to the total amount to cover credit card charges levied by the bank.
Card number, expiry date and name of card holder are necessary.
- Registrations will be confirmed by fax. Delegates will be requested to present letters of confirmation on-site.
- An on-site Registration Desk will be open at the Symposium. We strongly recommend, however, that participants pre-register early by mail. This will greatly facilitate registration procedures on site. Please note that the Secretariat will not accept pre-registration applications after August 28, 1997.
- Written cancellations received prior to June 30,'97 will be fully refunded. Thereafter, there will be no refund.
