• 1962年Arnold P. Friedman（1909〜1990, ニューヨークの内科医）らが中心となった米国神経学会・頭痛分類特別委員会の頭痛分類（Ad Hoc Committee on Classification of Headache）では15項目の分類を発表し、これが世界的に長く使われてきた。
  　この分類は、頭痛を「機能的な頭痛」と「基質的変化に基づく頭痛（症候性頭痛）」に分けていた。さらに、機能的頭痛を「血管性頭痛」と「緊張性頭痛」に分け、片頭痛や群発頭痛などを血管性頭痛としてまとめていた。

• Ad Hoc Committeeの分類↑では、片頭痛を血管性頭痛に分類するのは問題があるとする立場が生じ、Olesenらを中心に新しい頭痛の分類が試みられ、1988年、国際頭痛学会 International Society of Headacheが中心となり、新しい分類(ICDH)を発表し、さらに2004年に改訂 (ICHD-II)された(分類2)。
  　この新しい新分類は13項目から成る。「機能性頭痛である片頭痛、緊張性頭痛、群発頭痛」と「基質的疾患に基づく他の頭痛（症候性頭痛）」に分けられていて、この点では、Ad Hoc Committeeの分類と同じである。
